DENTAL PATIENT BILL OF RIGHTS


You have the right to...

  • Schedule an appointment with your dentist in a timely manner.
  • See the dentist every time you receive treatment.
  • Know in advance the type and expected cost of treatment.
  • Expect dental team members to use appropriate infection and sterilization controls.
  • Ask about treatment alternatives and be told, in a language you can understand, the advantages and disadvantages of each.
  • Ask your dentist to explain all the treatment options regardless of coverage or cost.
  • Know the education and training of your dentist and the dental team.
  • Know the professional rules, laws, and ethics that apply to your dentist and the dental team.
  • Choose your own dentist.

We thank you for choosing us to provide your dental care. We also look forward to the opportunity to serve you and we believe in extending the finest dental care available to our patients. In order to do that, we have found the following policies to be helpful.



FEES
Our fees are determined by the care, skill and judgement that the doctor exercises for a given procedure, together with the length of time that the procedure takes.



PAYMENT FOR SERVICE

Full payment for the dental treatment provided is expected at the time of service. For your convenience, we accept cash, checks, and major credit cards. For our patients who need an extended payment plan, we are pleased to offer a dental finance plan with a very competitive interest rate.



DENTAL INSURANCE

The amount paid to you by insurers is the amount of insurance coverage which you or your employer has purchased. These benefits are specified in your contract and bare no relationship to the value of our services. The phrase, "More than the allowable charge" or "Exceeds usual, customary amount" is the standard language used by the insurance industry to state that your insurance does not cover the full amount of the fee, and again bears no relationship to the value of our fees.



Patients who carry any form of dental insurance should know that all services furnished are charged directly to the patient and that he/she is personally responsible for payment. Your dental insurance policy is a contract between you (the policy holder) and the insurance company.



MINOR PATIENTS

Payment for the treatment of minor patients is the responsibility of the adult accompanying the patient regardless of any outside agreements or judgements.



APPOINTMENTS

Appointments must be canceled with a minimum of 48 hours advanced notice. Your cooperation will enable us to serve your needs as well as control the cost of services. If any appointment is not canceled 48 hours in advance, you may be charged for that missed appointment if the time allotted to you is not filled.
