We thank you for choosing us to provide your dental care. We also look forward to the opportunity to serve you and we believe in extending the finest dental care available to our patients. In order to do that, we have found the following policies to be helpful.
Our fees are determined by the care, skill and judgement that the doctor exercises for a given procedure, together with the length of time that the procedure takes.
PAYMENT FOR SERVICE
Full payment for the dental treatment provided is expected at the time of service. For your convenience, we accept cash, checks, and major credit cards. For our patients who need an extended payment plan, we are pleased to offer a dental finance plan with a very competitive interest rate.
The amount paid to you by insurers is the amount of insurance coverage which you or your employer has purchased. These benefits are specified in your contract and bare no relationship to the value of our services. The phrase, "More than the allowable charge" or "Exceeds usual, customary amount" is the standard language used by the insurance industry to state that your insurance does not cover the full amount of the fee, and again bears no relationship to the value of our fees.
Patients who carry any form of dental insurance should know that all services furnished are charged directly to the patient and that he/she is personally responsible for payment. Your dental insurance policy is a contract between you (the policy holder) and the insurance company.
MINOR PATIENTSPayment for the treatment of minor patients is the responsibility of the adult accompanying the patient regardless of any outside agreements or judgements.
Appointments must be canceled with a minimum of 48 hours advanced notice. Your cooperation will enable us to serve your needs as well as control the cost of services. If any appointment is not canceled 48 hours in advance, you may be charged for that missed appointment if the time allotted to you is not filled.
